Recovery Case Manager - Crisis Triage Center

Recovery Case Manager - Sunday, Monday, Tuesday and half day Wednesday.

General Summary of Duties: The Recovery Case Manager provides discharge planning functions for clients preparing to leave the Crisis Triage Center (CTC), including connection to local resources, services, and integrated health care

The Recovery Case Manager improves the operations of KMHS in support of its stated mission, and provides the highest level of customer service to internal and external customers.

Major Responsibilities/Tasks :
  • Collaborate with the multi-disciplinary team at the CTC, and to identify clients individual needs that need to be addressed in order to facilitate an efficient and effective discharge from the CTC. Those needs may include, but are not limited to: access to benefits, housing, linkages to medical and psychiatric/SUD services, transportation and other supports that will enhance the likelihood of clients having their essential needs met when they return to the community.
  • Assume responsibility in ensuring that clients are accessing the full range of entitlements for which they are eligible, to support in connection to housing resources, access to reliable transportation, and establish linkages to outpatient treatment programs both at KMHS and to outside programs when indicated.
  • Maintain and improve community contacts so that a referral to external providers becomes an easily navigated process. Initiate and maintain partnerships with other relevant systems to promote access to the full range of services and supports and to ensure continuity of care.
  • Provide ongoing problem-solving and practical assistance to clients in accessing the resources they will need when returning to the community.
  • Collaborate with the clients assigned outpatient clinician, the housing coordinator, the CTC team, all involved community agencies and families to ensure the success of the plan for discharge.

  • Act as liaison and consult with community agencies and families to maintain coordination in the discharge process.
  • Collaborate with Housing Solutions Team to develop and enhance housing resources that are linked to appropriate levels of mental health supports and services.
  • Assist the shift supervisor in organizing the activities on the day of discharge from the facility.
  • Participate in shift pass downs at the CTC.
  • Document client progress and discharge planning activities in clients care record according to established methods and procedures.
  • Work in a cooperative and collaborative manner as a team member. Assist in building positive working relationships with staff of all agency departments.
  • Support in milieu management tasks of the CTC as indicated/needed up to and including verbal crisis intervention and de-escalation.
  • Take appropriate measures to meet client daily basic needs for health and safety including but not limited to the following tasks: a) prepare and serve meals, b) conduct safety checks of the Crisis units, c) monitor clients according to the assigned levels of observation, d) provide transportation as directed by a supervisor, e) assist in maintaining a clean work environment, , and g) other duties as assigned by a supervisor.
  • Disclose limited client information as is necessary to all individuals involved in the discharge plans in accordance to agency policy and training. Disclosures will adhere to KMHS privacy and security standards and be logged in detail according to agency policy and procedures. Complete all required training and annual refreshers.

Minimum Qualifications:

EDUCATION: Bachelors Degree in Psychology, social services, or behavioral health field.

EXPERIENCE: Entry level (no prior related work experience)

Possess and maintain a valid driver's license with an acceptable motor vehicle report and reliable, insured transportation

LICENSURE: Agency Affiliated Counselor Registration.

Preferred Qualifications:

EXPERIENCE: Experienced (minimum 2 years of job-related experience)

Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.

The capacity to work well with children and families.

Performance Requirements:

Equipment Operated: Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.

Work Environment : Frequent mobility and/or sitting required for extended periods.

Mental/Physical Requirements : While performing the job duties, the employee is required to walk, sit, use hands and fingers, reach with arms, talk, or listen. Peripheral vision is also required for this position.
