Housing Support Specialist II - Pendleton Pace

Compensation: $22.67 to $27.78 per hour

Location: Bremerton, WA

Shift: Full-Time, Monday-Friday - 12:00 am to 8:30 am

Major Responsibilities/Tasks:
  1. Assist in coordinating, improving, and implementing agency treatment philosophy, program goals, and activities. Provide a therapeutic environment per program standards and supportive housing goals, including skills development and transportation of residents as required.
  2. Develop positive rapport and good working relationships with clients in the housing program.
  3. Perform the Foundational Community Supports assessment and recertification with clients, collect supporting documentation, and create person-centered client goals that may include education, training, coaching, resolving disputes, and advocacy.
  4. Support the implementation of the Permanent Supportive Housing Fidelity model
  5. Assist consumer learning and development of complex and straightforward skills through training, demonstration, and observation of cooking, cleaning, money management, symptom management, social interaction, and other recovery skills.
  6. Provide structure as needed for clients. This could vary case by case and include helping a client work out a weekly schedule for housework, taking a group shopping regularly, or working side beside a less-abled client giving prompts on specific house cleaning tasks.
  7. Encourage the development, monitoring, and completion of housing goals.
  8. Maintain contact with Crisis Response Team, housing team, and clinical team. Report salient observations of client mental status; conditions posing increased risk for clients, staff, or others; indications of deterioration of client stability; or improvement such that unit level of services is no longer necessary.
  9. Provide early identification and intervention for behaviors that may jeopardize housing
  10. Take appropriate action when clients are missing, ill, injured, or otherwise physically and/or psychiatrically impaired.
  11. Document unusual events on incident reports and consult with appropriate agency personnel regarding a required course of action.
  12. Participate in the development of transitional planning for residents graduating to more independent living situations.
  13. Demonstrate practical medication observation and documentation for consumers whose medications are being monitored.
  14. Work cooperatively and collaboratively as a team member. Assist in building positive working relationships with staff of all agency departments.
  15. Provide backup to staff in other programs as needed and when the schedule of duties and responsibilities allows.
  16. Communicate regularly with other KMHS assigned to each client's care regarding the client's needs and progress and emergencies such as decompensation, unsanitary or unhealthy living conditions, and exploitation of or by others.
  17. Document all services thoroughly and accurately and submit all documentation promptly per agency policy and WAC requirements.

Minimum Qualifications:

EDUCATION / EXPERIENCE: Bachelors Degree in Psychology, social services, or behavioral health field OR Four or more years relevant experience OR an equivalent combination of education and experience.
Possess and maintain a valid driver's license with an acceptable motor vehicle report and reliable, insured transportation.
LICENSURE: Agency Affiliated Counselor Registration or ability to obtain.

Preferred Qualifications:

EXPERIENCE: Experience in co-occurring disorders treatment, individual and group therapy, vocational services, or substance abuse treatment.
